Apply for a Passport for a Minor

The Consular Cashier accepts cash in U.S. dollars or equivalent Mongolian tugrug. Local Mongolian and international credit cards (excluding U.S. bank issued credit cards), personal checks and other country currencies are not accepted.

All minor passport applicants under the age of 16 are required to appear in person with both parents or their guardian(s) at the U.S. Embassy.  See more information below if one or both parents are unable to appear in person.  Appointments are available on Tuesdays between 1:00 pm. and 3:00 pm. and can be made online at the Embassy’s ACS Online Appointment System.  Follow the directions outlined on the Department of State website and submit all forms in person.

 

1. Evidence of U.S. Citizenship (current passport, or birth certificate if this is the minor’s first passport)

2. Evidence of Parental Relationship (birth certificate)

3. Parents identification (Passports or Citizen IDs)

4. One copy for each document.

5. Parental Consent (see directions below)

6. Age Progression Photos (if the previous passport was obtained when the child was a baby, please bring age progression photos of the child, year by year)

7. You must provide one photo of your child (2 X 2 inch (5cm X 5cm) photo on a white background is required). To avoid processing delays, be sure the photo meets all our requirements.

8. Completed and printed Application Form DS-11.

9. Non-refundable fee in cash USD or MNT equivalent. $135 ($165, if reached age of 16 and applying for the 1sttime Adult Passport).

Children under age 16 cannot apply for a passport by themselves.

Both parents/guardians must appear in person with the child and provide consent, authorizing passport issuance to the child. If one parent/guardian is unable to appear in person, then the DS-11 application must be accompanied by a signed, notarized Form DS-3053: Statement of Consent (PDF) from the non-applying parent/guardian.  The absent parent must also provide a notarized copy of the front and back side of the photo I.D. that was used during notarization. In certain countries, a DS-3053 must be notarized at a U.S. embassy or consulate and cannot be notarized by a local notary public. The document is only valid for 90 days after date of signature.

If the child only has one parent/guardian, evidence of sole authority to apply for the child must be submitted with the application in the form of a:

  • Court order granting sole legal custody to the applying parent (unless child’s travel is restricted by that order)
  • Court order specifically permitting the applying parent to apply for the child’s passport
  • Judicial declaration of incompetence of the non-applying parent
  • Death certificate of the non-applying parent

Photocopies and notarized copies are unacceptable

Special Circumstances

If you are the applying parent or guardian and the written consent of the non-applying parent or guardian or evidence of sole custody cannot be obtained in a timely manner, complete the DS-5525 “Statement of Exigent/Special Family Circumstances” (PDF 47 KB). The statement must explain in detail the non-applying parent’s or guardian’s unavailability and recent efforts made to contact the non-applying parent. You may also be required to provide evidence (e.g., custody order, incarceration order, restraining order) to document your claim of exigent or special circumstances.  The absent parent must also provide a notarized copy of the front and back side of the photo I.D. that was used during notarization.  The document is only valid for 90 days after date of signature.

Embassy does not provide service for Social Security Number.
